Complete Your Patient Registration Before Your Appointment

We want to ensure your visit goes smoothly. If you’ve scheduled an appointment and have not yet filled out your Patient Registration Forms, we kindly ask that you complete them before your appointment. You can download the necessary forms below.

If you’re unable to complete the forms in advance, please arrive 15 minutes early to allow time to fill out the paperwork. Our Front Office Representative will provide these forms when you check-in.

Please note that even if you’re an existing patient, we may request updated Patient Registration Forms depending on how long it’s been since your last visit. Additionally, when you arrive for your appointment, we will ask for a form of identification and your insurance card.

How to Request Your Medical Records

To request a copy of your medical records, please download and complete our Release of Medical Records Request Form. This form allows you to authorize the release of your records to or from Tucson Dermatology. Once completed, you can submit the form via mail, email, or fax to our office. Please ensure all sections are filled out accurately to avoid any delays in processing your request. If you have any questions or need assistance, feel free to contact our office. Our team is here to help facilitate a smooth and timely transfer of your records.
